Работа посвящена комплексному анализу экономической нагрузки шизофрении на системы здравоохранения Российской Федерации (РФ) и Республики Беларусь (РБ). В исследовании оцениваются прямые медицинские затраты, связанные с лечением и реабилитацией пациентов, а также рассчитываются потери производительности труда из-за инвалидизации и снижения трудоспособности больных. В ходе работы проанализированы эпидемиологические данные по шизофрении в РФ и РБ, оценены текущие расходы на стационарное и амбулаторное лечение, медикаментозную терапию и реабилитационные мероприятия. Особое внимание уделено изучению влияния программ поддерживающей терапии на снижение частоты и длительности госпитализаций, а также на повышение уровня занятости пациентов. Методология исследования включает статистический и сравнительный анализ данных национальных министерств здравоохранения, моделирование экономических показателей с использованием формулы расчета потерь производительности, а также обзор научных публикаций и результатов клинических исследований. Результаты показывают, что экономическая нагрузка шизофрении складывается из значительных прямых затрат (20-30 млрд руб. в РФ и 300-400 млн белорусских руб. в РБ ежегодно) и еще более существенных потерь производительности (40-60 млрд руб. для РФ и 500-700 млн белорусских руб. для РБ). Внедрение программ поддерживающей терапии может снизить общую нагрузку на 10-15 % за счет сокращения госпитализаций и повышения трудоспособности пациентов. На основе полученных данных сформулированы рекомендации по оптимизации финансирования психиатрической помощи, развитию амбулаторной поддержки и внедрению программ трудовой адаптации. Результаты исследования могут быть использованы органами здравоохранения РФ и РБ для планирования бюджета и совершенствования системы оказания психиатрической помощи.The work is devoted to a comprehensive analysis of the economic burden of schizophrenia on the healthcare systems of the Russian Federation and the Republic of Belarus. The study assesses direct medical costs associated with the treatment and rehabilitation of patients and also calculates productivity losses due to disability and reduced work capacity. During the research, epidemiological data on schizophrenia in the Russian Federation and the Republic of Belarus were analysed, and current expenditures on inpatient and outpatient care, drug therapy, and rehabilitation measures were estimated. Particular attention is paid to the impact of maintenance therapy programmes on reducing the frequency and duration of hospitalisations and on increasing patients’ employment rates. The methodology includes statistical and comparative analysis of data from national ministries of health, modelling of economic indicators using a productivity-loss formula, and a review of scientific publications and clinical trial results. The results show that the economic burden of schizophrenia consists of significant direct costs and even more substantial productivity losses. Implementation of maintenance therapy programmes can reduce the overall burden by 10-15% through fewer hospitalisations and improved patient work capacity. Based on the findings, recommendations are formulated to optimise financing of mental healthcare, develop outpatient support, and implement work adaptation programmes. The research results can be used by healthcare authorities in the Russian Federation and the Republic of Belarus for budget planning and improvement of psychiatric care systems.
